Instructions for the Power of Attorney Form:
Power of Attorney Form
In order for WWR's customs broker to clear your shipment(s) through U.S. Customs, a completed and signed Power of Attorney form must accompany the 3299, Supplemental Declaration and passport copies. The Power of Attorney enables WWR's customs broker to complete and sign these forms on your behalf. Any additional information required from you will be obtained by telephone or fax.
The form will be kept on file with the customs broker.
WWR will complete all the necessary information on this form. Please simply sign and date the form where indicated at the bottom.
